What services do you offer?
We offer integrated primary care and mental health services for adults and adolescents. Our team provides annual wellness visits, chronic disease management, preventive care, and treatment for conditions such as anxiety, depression, and ADHD. We also collaborate on personalized care plans that may include medication management, therapy referrals, and lifestyle support.
Who do you treat?
We provide personalized, person-centered care for teens, adults, and seniors; however, we are not accepting children under 12 at this time.

What if I don’t have insurance?
We offer self-pay options and are happy to discuss current pricing and available payment methods. At the discretion of practice leadership, we may also work with patients to establish mutually feasible payment plans; in some cases, proof of financial hardship may be required.

What is your cancellation policy?
